Where we suggested areas of improvements to practice the service was responsive. We also found people were protected from the risk of social isolation through regular encouraging interactions by staff and the service had an activities co-ordinator in place. This report only covers our findings in relation to the Key Questions of Safe and Well-led which contain those requirements. The last rating for this service was requires improvement and there were breaches of regulation. The provider completed an action plan after the last inspection to show what they would do and by when to improve. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests.

It is an annual review prepared by CQC after examining previous reports and information from the provider. At the time of this report, CQC judged the service to be Good. Care plans were not always in place or updated in relation to known risks to people. Audits had not always identified issues with medicine records. We had not always been notified about events at the service as required.

We were concerned that the service was failing to notify us of required matters. As a result, we undertook a focused inspection to review the key questions of safe and well-led only. We found that people who used the service were protected and safe. We found that there was an effective infection control system in place and that the home had a clean and suitable environment.

Accidents and incidents were appropriately recorded and risk assessments were in place. The registered manager understood their responsibilities with regard to safeguarding and staff had been trained in safeguarding vulnerable adults. This inspection report is written in a shorter format because our overall rating of the service has not changed since our last inspection. West House Care Home Limited accommodates 26 people in one adapted building, across two floors. At the time of the inspection, there were 23 people using the service.

There were sufficient numbers of staff on duty in order to meet the needs of people who used the service. The provider had an effective recruitment and selection procedure in place and carried out relevant vetting checks when they employed staff. Staff were suitably trained and received regular supervisions and appraisals. The Provider has not yet updated their profile and added details of the services and facilities they offer. This report only covers our findings in relation to the Key Questions of Safe and Well-led which contain those requirements. The last rating for this service was requires improvement and there were breaches of regulation. The provider completed an action plan after the last inspection to show what they would do and by when to improve. People were supported to have maximum choice and control of their lives and staff supported them in the least restrictive way possible and in their best interests.

The service used a keyworker system and we found that staff had a comprehensive knowledge of people’s preferences, needs, likes and dislikes. West House is a residential care home in Chester-le-Street providing accommodation, personal care and nursing care for up to 26 older people. It is registered to provide nursing and residential care for up to 30 older persons. The home offers single and double room accommodation, with communal sitting/dining areas. Bathrooms are located throughout the home, some of which have aids to support people during personal care. West House is situated in Chester-le street, a town near to Durham and Newcastle.

We saw staff understood each person’s different needs, for example, when they required additional support with their mobility. People were supported to have maximum choice and control of their lives, and staff supported them in the least restrictive way possible; the policies and systems in the service supported this practice. We saw staff stand over people when assisting them with meals, which did not promote people's dignity. We saw the provider had made suitable adaptations to meet the people's physical needs.

Latest Inspection

Although this home catered for people whose primary need related to general nursing, some people also had dementia. These people, because of their dementia, were not able to tell us what they thought about living in the home. We saw that staff talked with people in a respectful manner and had a good understanding of their communication and psychological needs. However, at meal times we saw that staff did not always sit beside the people they were supporting. We saw staff stand over people when assisting them with meals, which did not promote people’s dignity. We found that staff had been well supported to deliver care and treatment safely.

Care records showed that people’s needs were assessed before they started using the service. At other times we offer an answer phone service so please leave a message and we'll call you back. We will continue to monitor information we receive about the service until we return to visit as per our re-inspection programme.

Reviews

Daily help, including personal care, is available on site and one or more meals are available daily in a dining room or restaurant. Additional facilities are also common – for example hairdressing salons, hobby rooms and gyms. Organised activities will focus on helping residents maintain their health and wellbeing. At the previous inspection on 20 and 22 October 2015 we found the service did not have in place personalised activity plans which met people’s needs and reflected their preferences. This was a breach of Regulation 9 of the Health and Social Care Act Regulations 2014. People were protected from the risk of poor nutrition and staff were aware of people’s nutritional needs.

If we receive any concerning information we may inspect sooner. Full information about CQC’s regulatory response to the more serious concerns found during inspections is added to reports after any representations and appeals have been concluded. People spoke positively about the leadership of the manager and said communication from the provider was good.

West House Care Home Limited, Waldridge Road, Chester Le Street.

A new fire risk assessment had been carried out and staff had completed fire safety training. The provider was renovating the home, new bathrooms and shower rooms had been installed and areas of the home had been redecorated. Quality assurance systems were effective, but the registered manager had plans to make further improvements.
